There is quite a bit of common information for registering for status, the Secure Certificate of Indian Status and the Certificate of Indian Status. Please note, these are guidelines only for a straightforward application – individual circumstances may vary (name changes, custody arrangements, etc.) and more information/documentation may be required:

For Registration and/or Secure Certificate of Indian Status:

An individual 16 and over is wanting to apply for registration and the Secure Certificate of Indian Status (optional) for themselves, they will need:

  • Form 83-168SE – APPLICATION FOR REGISTRATION ON THE INDIAN REGISTER AND FOR THE SECURE CERTIFICATE OF INDIAN STATUS (SCIS) (Adults 16 and over)
  • Form 83-168IE – INSTRUCTIONS FOR THE APPLICATION FOR REGISTRATION ON THE INDIAN REGISTER AND FOR THE SCIS. (Adults 16 and over)
  • Form 83-169SE – GUARANTOR FORM & INSTRUCTIONS (form to be completed by an eligible guarantor)
  • ORIGINAL proof of birth document that includes their parents names. The original will be submitted with the application.
  • 1 piece of VALID identification that includes the applicants: Full name, DOB, Photo & Signature (PP, DL, HC). Photocopy of front and back signed by the guarantor.

A parent(s)/guardian is wanting to apply for registration and the Secure Certificate of Indian Status (optional) for their child 15 and under, they will need:

  • Form 83-171SE – APPLICATION FOR REGISTRATION ON THE INDIAN REGISTER AND FOR THE SECURE CERTIFICATE OF INDIAN STATUS (SCIS) (Children 15 and under)
  • Form 83-171IE – INSTRUCTIONS FOR THE APPLICATION FOR REGISTRATION ON THE INDIAN REGISTER AND FOR THE SCIS (Children 15 and under)
  • Form 83-169SE – GUARANTOR FORM & INSTRUCTIONS (to be completed by an eligible guarantor for the applying parent, not the child.)
  • ORIGINAL proof of birth document for the child that includes the parents names.  The original will be submitted with the application.
  • 1 piece of VALID identification (for the parent) that includes their Full name, DOB, Photo & Signature (PP, DL, HC). Photocopy of front and back signed by the guarantor.

*for a child application, ONE parent may be the applicant but BOTH parents are required to sign the application unless there is documentation in place proving sole custody to the applying parent.

An individual is already registered, and is wanting to apply for the Secure Certificate of Indian Status (all ages) they will need:

  • Form 83-172SE – SCIS APPLICATION FOR ALREADY REGISTERED PERSONS
  • Form 83-172IE – INSTRUCTIONS FOR SCIS APPLICATION FOR ALREADY REGISTERED PERSONS
  • Form 83-169SE – GUARANTOR FORM & INSTRUCTIONS (completed by an eligible guarantor for the adult applicant OR applying parent.)
  • 2 Canadian passport-style photos of the person the SCIS is requested for –  one of which needs to signed by the guarantor.
  • 1 piece of VALID identification (for the adult applicant OR the applying parent) that includes Full name, DOB, Photo & Signature (PP, DL, HC, SCIS). Photocopy of the front and back signed by the guarantor.
  • Custody documents or Name-linking documents if applicable.

*current processing time for registration is 6 months to 2 years depending on complexity and 16 + weeks for an SCIS-only application.

For NAN band members wishing to apply for the laminate CIS: (most common)

*Note: ID must be original and valid; photocopies are not acceptable and laminate CIS cannot be issued through mail.

A NAN band member, 16 and up, is already registered and is wanting to apply for a laminate CIS for themselves, they must have:

  • 1 piece of Primary ID (passport) OR
  • 2 pieces of Secondary ID (Drivers Licence, Health Card, laminate CIS, Provincial ID card, Employee or Student ID with digitized photo, Firearms licence, Birth certificate) OR
  • 1 piece of Secondary ID PLUS a guarantor form completed by an eligible guarantor.
  • Cards are issued on an appointment-only basis by calling Bimose 468-5551 or e-mail [email protected] – forms will be provided at the appointment.

A NAN band member is wanting to apply for a laminate CIS for their child, 15 and under, they must have:

  • 1 piece of Primary or Secondary ID for the parent.>
  • 1 valid piece of ID for the child (Birth certificate, health card.)
  • A copy of any documentation pertaining to custody of the child, if applicable.
  • Cards are issued on an appointment-only basis by calling Bimose 468-5551 or e-mail [email protected] – forms will be provided at the appointment.
